Tier 1 — Essential Care | $350/month

Foundational concierge care covering medication management, remote patient monitoring, chronic disease support, and monthly telehealth visits.

• Medication management and reconciliation

• Remote Patient Monitoring via Hume Health device (device sold separately)

• Chronic disease monitoring: diabetes, hypertension, heart disease, high cholesterol

• Monthly telehealth visit with Joy

• Monthly provider coordination update

• Disease process and nutrition education

• Strength and fall prevention teaching

• Oxygen safety and home maintenance education

• Access to online patient and family education resources

Tier 2 — Comprehensive Care | $500/month

Everything in Essential Care, plus more frequent visits and post-hospitalization support.

• Everything included in Tier 1 — Essential Care

• Second monthly telehealth visit (2 total per month)

• 1 in-person visit per month

• Post-hospitalization medication assessment and reconciliation

• Increased provider coordination and communication

• Family and facility staff support and education

Tier 3 — Advanced Concierge Care | $700/month

The most comprehensive tier — everything in Tiers 1 and 2, plus additional in-person visits, priority access, wound care, and full care coordination.

• Everything included in Tiers 1 and 2

• 1 additional in-person visit per month (2 total per month)

• Priority scheduling for as-needed visits

• Wound assessment, prevention, and management

• Wound care visits (frequency based on clinical need)

• Medication self-administration assessment (ALF/ILF residents)

• Comprehensive care coordination across all providers

À La Carte Nursing Visits

$150 per visit — available as an add-on with any tier, or as a standalone service for families not yet enrolled in a monthly plan.

Private Pay Add-On Services

Specialized nursing services available to enrolled clients based on clinical need:

• PICC line management

• Colostomy management

• Catheter management

• Wound VAC management

• Wound care teaching

• Wound care supplies

Remote Patient Monitoring Devices

Hume Health devices are available for one-time purchase and enable continuous health monitoring between visits:

• Hume Body Pod — $229

• Hume Band — $249

Devices are required for Remote Patient Monitoring included in all concierge tiers.
